How to ask your employer in writing for a workplace change related to a medical condition or other protected need.
A written request creates a clear record of what you asked for and when. This guide is general information, not legal advice. Deadlines and rules differ, and using Report 2 HR does not stop or extend any deadline. See Official help for agencies.
Say what you need
Describe the change you are asking for, such as a different schedule, a seat, or a break. You are not required to use special words.
Explain why only as much as you choose
You can describe the connection to your condition. Think about how much medical detail you want in writing; you might provide more only if the employer asks and you decide to.
Give the date and keep a copy
Date your request and keep a copy of it and any replies.
Follow up
If you receive no reply, write a short follow-up and note the date.
Fictional sample: request for a schedule change
This sample is fictional. Names and events are made up.
On September 5 I am asking for my Tuesday and Thursday shifts to start at 10:00 am instead of 8:00 am because of a medical appointment schedule that I can explain if needed.
I would like a written reply by September 19. I am open to talking about other ways to make this work.
